Transition to new EHR System- Elation Health and Patient Portal – Elation Passport

Thank you for your patience as Phinney Primary Care & Wellness (formerly known as Fremont Family Practice) transitions to a new electronic health records system – Elation EHR.

The new EHR comes with an upgraded patient portal called Elation Passport. You will receive an email invite to join the Elation Passport. If you have not received an email yet and would like to join Elation Passport now, please send an email to Amna at officemanager@phinneypcw.com to send you an individual invitation.

Access to Patient Fusion – If you are enrolled in Patient Fusion account through our previous EHR, you can send messages to your providers up until May 20 2023. All your medical records will be transferred to the Elation Passport patient portal. 

Although we started using Elation on May 15 2023, the transition can take up to 4-6 weeks to complete. There are various integrations that need to be in place for the system to work efficiently. We appreciate your patience our providers and staff during this period of time. We apologize for the inconvenience this may cause over the next few weeks.
