Online Scheduling
When you schedule online, we need to know the following information:
(1) The reason for your visit
(2) Do you want in office or telemedicine visit
(3) COVID-19 Vaccine status
Also, please note that new patient wellness exams require 2 adjacent time slots.
Dr. Grote is available for Telemedicine visits only on Tuesdays. Please do not request in office visits for Tuesdays, they will be denied automatically.
First Visit
New patient paperwork will be sent electronically to your email. Please complete this prior to your appointment arrival.
Please arrive no more than 10 minutes prior to your appointment so we may limit contact in the waiting room.
Additionally, please bring,
- ID card
- Insurance card
- A complete list of your medications
